HeartThrive Program

The HeartThrive Program is usable and viewable via a PDF viewer, such as Adobe Acrobat, and does not require you to install any additional software on your devices. 

It is completely portable and transferrable from one device to another, as it was created to be able to easily interface with various electronic health record (EHR) and electronic medical record (EMR) systems, and it is ready to be used on the go at any time.

We help you customize the written (clinical and non-clinical) content, layout design, color scheme, and logo design (to insert your brand’s existing logo).  We add what you need for your site and subtract what you don’t need, so essentially everything in each form is customizable!

Yes!  All forms in the HeartThrive Program have been carefully designed to include the most relevant information needed to manage CHF patients in an outpatient or ambulatory care setting.

You will have customized CHF clinical forms for your practice within 5 weeks (usually less!) of our initial 1-on-1 customization meeting.  This includes the time it takes to conduct our customization meeting, design the customized forms, and revise the forms (if needed) before your product launch.

Our HeartThrive Program was designed with you and your practice in mind!  Not only do we offer a collaborative practice agreement (CPA) template for you to initiate collaboration with clinics of your choice for CHF management (if you don’t already work with one), but all of the CHF clinical forms and toolkits in our program are also set up for you to effectively manage CHF patients long-term in collaboration with clinics and physicians, help physicians and clinics meet their CMS metrics, and streamline billing processes.

As part of the HeartThrive Program, we will provide a 1-2 hour program onboarding session to train your pharmacists and other relevant staff members on how to integrate into their daily workflow the customized CHF clinical forms and toolkits we create for your site-specific practice. This will take place in the form of a virtual training session conducted online.
